You understand that this authorization will remain in effect until you cancel it in writing, and you agree to notify sales organization in writing of any changes in your account information or termination of this authorization at least 30 days prior to the next billing date. This payment authorization is for the type of bill indicated above. You certify that you are an authorized user of this credit card or bank account and that you will not dispute the scheduled payments with your credit card company or bank provided the transactions correspond to the terms indicated in this authorization. You agree that the sales organization is authorized to contact you via phone, text or email regarding payment for fees.
